A second quarter 2007 report by CB Richard Ellis observes a rapidly tightening industrial market in Metro Denver. How has GlaxoSmithKline developed such a strong latestage pipeline? Will the changes made over the years in the company's R&D strategy sustain that level of development? It is all about R&D strategy. A few years ago it become obvious to us that R&D productivity was eroding in the industry. Everybody was looking at fewer and fewer new chemical entities being discovered and approved. At the same time, the investments in R&D were increasing dramatically. The whole industry in 1980 spent about billion in R&D, all companies included. That year, the FDA approved close to 30 new chemical entities. Then move 20 years forward, and in 2000 the industry spent billion on R&D, 13 times more than in 1980. And yet that year, FDA only approved 30 molecules as well. So we can see the problem. There are multiple reasons why the industry seems to be discovering fewer molecules of major significance. We decided to go back to the R&D process, and we changed pretty much everything. We changed the basic technologies, we embraced genomics to discover new targets, we embraced high-throughput technologies to be able to screen millions of compounds at a very fast pace, we changed the organization because R&D departments grew significantly with investment in R&D, and instead of 2, 000 scientists organizing a very classic pyramid type set up, we ended up with 18, 000 or 20, 000 scientists. That became hugely bureaucratic, so we had to reinvent the model of organization. We created multiple centers of excellence -- a center of excellence is targeting one or two critical diseases -- and those groups of 300 or 400 scientists have tremendous autonomy, responsibility, and transparency. Context: Symptoms of fluid retention in GH-deficient patients during GH replacement are greater in men than in women, suggesting that testosterone may augment or estradiol may attenuate the antinatriuretic actions of GH. The mechanisms underlying the sodiumretaining effects of GH are poorly understood. Aim: The aim of this study was to investigate the effects of GH and testosterone, alone and in combination, on extracellular water ECW ; and the hormonal mechanisms involved. Design: Two separate, open-label, randomized, two-period, crossover studies were performed; the first compared the effects of GH alone with those of GH and testosterone, and the second compared the effects of testosterone alone with those of GH and testosterone. Participants: Twelve hypopituitary men with GH deficiency and hypogonadism were studied. Intervention: During the weeks of intervention, GH 0.5 mg d ; and testosterone enanthate 250 mg ; were administered by im injection. Outcome Measures: The outcome measures were ECW, IGF-I, plasma renin activity PRA ; , aldosterone Aldo ; , and atrial natriuretic peptide ANP ; . Results: GH treatment significantly increased P 0.05 ; both IGF-I and ECW, and these changes were enhanced by cotreatment with testosterone P 0.07 for both ; . PRA, Aldo, and ANP levels did not change. Testosterone treatment alone did not change the IGF-I concentration, whereas cotreatment with GH induced a marked increase. Testosterone alone increased P 0.05 ; ECW, and the effect was augmented P 0.01 ; by cotreatment with GH. Although PRA and ANP did not change, plasma Aldo decreased after single and combined treatments. Conclusion: GH and testosterone exerted independent and additive effects on ECW. The mechanisms of fluid retention for both hormones are likely to be exerted on the renal tubules. This is the first direct evidence that testosterone increases ECW. Days in vitro expansion. To investigate the aging of MSC morphologically, we studied the culture for the presence of any abnormalities in cell morphology. Production of debris in the medium and granules in the cytoplasm has been used as markers for fibroblast aging in vitro[14]. In the present study, granulated cytoplasm and debris was observed and increased from the third month on. We investigated another marker for cellular senescence, normally the mean telomere length. In the absence of the enzyme telomerase, telomeres gradually get shorter as cell division proceeds, and shortening of telomere throughout the life span is well-documented[15]. The consequence maybe the inactivation of genes closest to the telomere sequences, either directly or indirectly by a position effect, perhaps involving the formation of heterochromatin. However, maintenance of telomere length has been observed in germ cells, some stem cells and malignant cells due to activation of the telomerase enzyme[16]. Our present study shows that MSC senescence is associated with telomere shortening during in vitro expansion. The rate of telomere shortening was 100 bp in every two passage. Other studies[17, 18] have shown that the telomere length of MSC shortens in culture expansion. However the rate of telomere shortening in Kassem et al[11] study was 100 bp PD, which is very high in comparison to our study. This difference maybe due to the long period of their culture 500 days ; in comparison to our study 120 days ; . They showed that mTRFL decreased from an average of 10.4 kb in early passage cells to 7.1 kbp in late passage cells. Although loss of mTRFL in later passages is high but the mean amount of shortening was divided by the cumulative PD number. This could partly explain the difference. In addition to the telomere length shortening, we investigated the osteogenic and adipogenic differentiation capacity of MSC. MSC differentiation into other lineages has been used as a marker for the multipotential nature of these cells[19]. In our study, the differentiation potential to adipocyte and osteocyte dropped in the late-passages.
Experimental evidence that BRL acts indeed as a K -channel activator. Effects of BRL on Rat Portal Vein: Evidence for Increased K + Permeability At concentrations above 0.1 \xM, BRL stimulated M Rb + efflux from the portal vein Figure 2 ; , which reflects an increase in the K + permeability of the smooth muscle membrane. This activity of BRL resided primarily in the -- ; -enantiomer, with the + ; -enantiomer increasing flux only at concentrations 100 times higher. Additional support for the notion that BRL acts by increasing the cell membrane K + permeability stems from the particular pattern by which spontaneous activity of the portal vein was inhibited. As shown in Figure 2, BRL first decreased the frequency of the contractions without reducing their amplitude. This is typical of hyperpolarization of a pacemaker, by activation of K + channels after a burst of activity.27 It is known that spontaneous activity in the portal vein originates in some electrically unstable regions i.e., pacemaker regions from where the excitation is propagated electrically along the vessel ; . For a review, see Ljung.28 ; A preferential action of BRL on the pacemaker regions of the portal vein perhaps due to the particular sliding membrane potential there ; would also explain why stimulation of Rb * efflux is observed only at concentrations higher than those required to decrease the frequency of spontaneous activity. If low concentrations of BRL primarily increase the K + permeability of the pacemaker cells, this will inhibit myogenic activity by decreasing the frequency without producing a measurable increase in overall Rb * efflux, since and emend.

Believe me, people are not knocking down doors to be able to volunteer their lives for the service of others. How many would drop whatever they are doing to speed to a motor vehicle accident that may be a stranger, or it may be their neighbor or a family member? How many would risk their lives and health to put out a fire? And all of this for absolutely nothing but the personal satisfaction of helping others. Our focus in dermatology is on the treatment of two very common diseases--the inflamed skin condition known as atopic dermatitis, or eczema, and fungal nail infections. Novartis offers a series of leading medicines for these conditions. Elidel is the first and only non-steroid cream for eczema, a disease that affects about 10% of children in the US, while Lamisil tablets are the most frequently prescribed treatment worldwide for fungal nail infection. Key Marketed Products Elidel pimecrolimus ; was the first non-steroid cream approved for the treatment of atopic dermatitis, a skin condition commonly known as eczema, in adults and children. It is one of the first new eczema treatments introduced since the 1950s, when topical corticosteroids--historically the mainstay of therapy--became available. First launched in 2002 in the US, Elidel is now registered in approximately 90 countries, including many EU markets. Following discussions with the FDA, prescribing information for Elidel dispensed only as a topical cream ; will be updated in early 2006. A boxed warning and medication guide make clear that no causal link has been established between the use of Elidel and rare post-marketing reports of malignancy. The concern of the FDA for a potential risk for malignancies exists based on the use of oral calcineurin inhibitors at high doses. A similar change in labeling will be made for other products in this class. While we believe this action in not substantiated by scientific or clinical evidence, we have agreed to make the requested changes and will communicate them to physicians and patients so that they can continue to use Elidel as labeled to effectively manage eczema. We are confident in the safety and efficacy of Elidel, which is one of the most thoroughly researched dermatology products in the world and continues to be supported with significant ongoing clinical trials. Foradil formoterol fumarate ; is a long-acting bronchodilator that offers onset of action within five minutes and 12 hour relief of symptoms for patients with asthma and COPD, which includes chronic bronchitis and emphysema. It was first approved and launched in Switzerland in 1994, followed by other key European markets in 1997. US approval was granted in 2001, and in 2002 we licensed Foradil to Schering-Plough in the US. We continue to market and distribute the product in other areas of the world. Foradil Aerolizer is a single-dose dry powder inhaler, while a metered-dose inhaler is available in some countries. This product is licensed from Yamanouchi. Lamisil terbinafine ; is a leading therapy for onychomycosis, also known as fungal nail infection. Lamisil tablets kill the fungus that causes the infection at its source, working through the patient's bloodstream. This product was first launched in 1991 and is now available in more than 90 countries, with the US the leading market. Lamisil tablets are also approved for treating athlete's foot tinea pedis ; and fungal infection of the scalp tinea capitis ; in some countries, though not yet in the US. Our Consumer Health Division's OTC Business Unit markets over-the-counter cream formulations of Lamisil for use in treating athlete's foot in many markets, including the US. Generic forms of terbinafine were launched in a number of European markets in 2005, with generic competition expected in the US in 2007 following the expiry of patent protection. Xolair omalizumab ; is the first humanized therapeutic antibody for the treatment of allergic asthma and the first approved therapy designed to target the antibody IgE, a key underlying cause of the symptoms of allergic asthma. Xolair is the only asthma treatment dosed every two or four weeks and is administered by subcutaneous injection. In the US, EU and Canada, Xolair is indicated for use in adults and children over age 12 with moderate-to-severe allergic asthma whose symptoms are inadequately controlled with inhaled corticosteroids. Xolair gained US and Australian regulatory approval in 2003, while EU approval was given in October 2005 for treating adults and adolescents with severe persistent allergic asthma that is inadequately controlled by current medication. This product is being jointly developed with Genentech and Tanox, and is co-marketed by Novartis in the US with Genentech and elidel.

For a pesticide to be included on the PIC list a country must nominate the chemical for inclusion. This can occur by two countries taking regulatory actions banning or restricting the use and sending this action along with health assessments to the PIC Secretariat or a developing country experiences under typical use of a chemical that is banned in most developing countries and this information is sent to the PIC Secretariat. If the Secretariat determines that the criteria for listing the chemical is met then a Decision Guidance Document DGD ; is developed that includes the regulatory actions taken and risk determinations which is sent to the Conference of Parties who decides whether the chemical is to be added to the PIC List. The DGD is then sent to all countries that belong to the Party and the individual country decides what action they want to take with the pesticide.7 There are three categories on the PIC: List; banned, severely restricted and severely hazardous pesticide formulation. A Banned pesticide is a chemical all uses of which within one or more categories have been prohibited by 19 and enbrel.
